Flu shot clinic for employees

The Rock Island County Health Department will offer flu shots for employees from 9 a.m.-2 p.m.

No appointment is necessary.

Bring with you:

• A copy of your insurance card (front and back) 

• Completed consent form (Forms will be available if you forget to bring one.)

We recommend that you call the phone number on your insurance card to make sure the Rock Island County Health Department is in-network and that the flu vaccine is covered, especially if you have out-of-state insurance.

If you do not have insurance, or your insurance is not accepted, the cost is $35. Cash or check is accepted.
