Petition to Committee on Advanced Standing and Degrees Paper form for former students only. In most cases, students requesting a waiver or substitution who are not currently enrolled will need to provide the same advisor support and supporting documentation as enrolled students. Petitions may not receive consideration without these items. If you have question about the Committee see the policy website for details. Students should arrange to have materials sent to:
- Advisor support send to Registrar Office
- Medical documentation send to Dean of Students Office
- Personal or family emergency documentation send to Dean of Students Office
- Any other supporting materials send to Registrar Office
Petition to Transfer Coursework Form for former students only. Current students must use online form.
